New Patient Registration

Please call in at reception to complete the registration forms. You will need to supply an acceptable form of identity in order to register which includes: driving licence, passport, birth certificate, tenancy agreement, utility bill, council tax bill or bank statement, in order to help prevent NHS fraud.

As a patient it is your responsibility to cancel an appointment that is no longer required. Clinical time is precious and could be used for another person who urgently needs to see a clinician. The Practice reserves the right to refuse advance-booking appointments to those patients who fail to attend appointments.

We operate a zero tolerance approach to violence. Our staff are here to help you but we will not tolerate abusive behaviour and will request that you be deducted from our register informing the Health Authority of the reasons.
