Self Referral
Self Refer to Physiotherapy
If you are suffering from musculoskeletal problems such as neck pain, back pain, muscle strains, ligament strains and arthritis you can self refer without having to contacting the Practice.
Unfortunately self-referral is not open to patients with neurological conditions such as Multiple sclerosis, Parkinsons, Stroke and respiratory conditions. For treatment of these conditions you would have to be referred by the Practice after consultation who will refer in the normal way to the appropriate Physiotherapy service.
If you would like to self refer please click on the link below and complete the online referral form.
